Fees to be paid to the local registrar of vital statistics are
as follows:
(1) No
fee shall be paid for registering and recording certificates of birth,
death and stillbirth.
(2) Certified
copy of death certificate:
(A) Twenty-one dollars ($21.00) for the first or only copy requested.
(B) Four dollars ($4.00) for each additional certified copy of the same
record requested in the same request.
(3) Certified
copy of birth record: Twenty-one dollars ($21.00), plus a two-dollar
surcharge, for a total of twenty-three dollars ($23.00).
(4) Wallet-sized
certification of birth: Twenty-one dollars ($21.00), plus a two-dollar
surcharge, for a total of twenty-three dollars ($23.00).
(5) When
a search of birth or death records is requested, a search fee of ten
dollars ($10.00) will be charged for death certificates and a search
fee of ten dollars ($10.00), plus a two-dollar surcharge, for a total
of twelve dollars ($12.00), will be charged for birth certificates.
If the birth or death record is found and a copy purchased, then the
search fee shall not be assessed.
